Shared Care Record

The Ngaio Medical Centre is considering making a 'Shared Care Record' available. This is a programme run through a website called ManageMyHealth.  This is a secure website that receives an individual's personal health information uploaded from the practice management system.  Only registered health professionals in approved care settings will be eligible to access such records.  Access will be granted on a case-by-case basis. It can also be used to store other health related information that might not be recorded, such as treatment you have received, or other medications that you may be taking.  This allows some information to be available to care settings such as hospital in-patient or out-patient departments, emergency departments and after hours medical centres.  Such information would include medications, classifications (for instance, past diagnoses), allergies, laboratory results and discharge letters.

Consultation notes will not be shared.

Quoting their website, 'ManageMyHealth ... has a number of other features including health related news, community forums and access to wellness initiatives.  Over time, there will be more features available within the site providing users an online health resource to assist in the management of both their own, and their family's healthcare and wellbeing.'

Patients will be able to book appointments online, leave a secure message, view laboratory results, order repeat prescriptions and request email consultations through a planned portal system..

If patients wish to know if they have a Shared Care Record available to approved health providers, or wish to request details of who has accessed their records, they can ring the Shared Care Record freephone number - 0800 727 664 -  or check with the practice.  By contacting the practice or phoning the number, one will be able to opt out of the programme. Also, patients can remain in the system but can have specific classifications and medications withheld.

Those under 18 will have a record available in the 'Provider Portal'. Patient Portal access will be restricted to those over 18 and only for those practices that wish to offer this functionality.

If anyone wishes to raise a complaint about this project, they should follow the standard complaints process through the Patient Advocacy Service. Patients may also make a complaint by ringing the dedicated Shared Care Record freephone number 0800 727 664.
